Service Learning Forms

Reminder: students are required to complete 5 hours a semester of service learning. No pre-approval of your activity is required as long as it is community service and you are not receiving compensation. 

When you complete your requirements you must submit this form. Due no later than the Friday before grades are due each semester to the dropbox on Moodle (in courses Nurs 250, 252, 350, 352, 450B, and 455)
